Make sure that your new host gives you a temporary domain. Freeze your live site, then transfer files/DB over to the new site and configure for the temporary domain. Transfer the domain, then reconfigure for the new URL ASAP.
Remember to backup your database right away too...very important...

As mentioned by others , take a back-up of both the source code as well as the database dump.
Then put-up a banner at the top of the front page in your site which is clearly visible for the visitor , that there will be site maintanence & the site will return back to normal within these many hours (you have to evaluate the time by discussing with the new hosting provider & the tech guy , whose going to help you migrate the site)
Its better to inform the visitors about the migration of the server , so that you don't lose business or returning customers.
