Featured Site Section on PageOne

Who decides these featured sites?

Do we have to submit a site or is it simply chosen?

They are usually selected from the site review forum by one of the Admins. I think they try to pick sites that have not had many reviews.

Or, sometimes after one of the contests we have, they'll list the winner's site as featured.

Yep You are right flood6, it pretty much is random who gets picked. Like you said depends on how many posts it gets, what type of site it is, etc... No real method to the madness.
