Need help creating an order form

I am creating a website for someone. They have over 5000 items to sell. They want the products listed like a catalogue with a check box so that customers can click on the items they want to order. I am going to have to make 100's of these lists. What I am trying to make is an order form for the products they have chose to be sent to, where the customer will add their name and address and then it can be emailed to the company. Can anyone help me figure this out? I have no idea how to get all the items they want to order to one page.

My advice would be to store the item data in a database, then automatically generate the pages from that based on product categories (you really can't have 5000 items on 1 page).

As the customer adds items to the cart you store the customers selections in the database, once they get to the end they enter their name etc, and you can transfer all the data to the seller.

There is a free script at http://www.oscommerce.com/ which requires php with a mysql database, it requires no programming skills once setup. Comes with a large amount of features.
