Need A Web Based Contact Database

I am looking for a database that is web based, very clean and easy to use. the backend will store customer information such as name, phone, services offered, email, zip codes serviced, and maybe two or 3 other fields

the front end will need to be password secured and allow for a user to access a website and enter requested service and zip code or state, thus pulling up the customers info from the back end...
