Gathering data for an affiliate survey

Have you ever wondered which solution providers are most popular with affiliates? How many programs the average affiliate promotes? How many affiliates have successfully asked for increased commissions?

I believe there is an issue with the survey, second page, 1st question.

You can only select 1provide as Average, 1 as High, and 1 low. There is also the same problem with the second page, second question.

Otherwise great survey! I look forward to seeing the results.
