Hi,
I recently asked forum members to take a look at a custom landing page I was designing for an Adwords campaign. I’ve made changes based on the comments (taken out CAPTCHA, required fields etc) and would really appreciate any new comments. Links below...

I like the new landing page but would recommend one thing.
You might consider adding a line of text or a graphic which reads something to the effect of "DVD's for just $1.49 ea." or whatever your base price is on a set order size.
Link it to your products & pricing page to explain the requirements and conditions.

Much better. One thing about the Quantity field..... I would suggest you not put a open field there. The reason is that it isn't text, but a quantity. I would put numeric ranges. This way, if your campaign is large, later you can go back and analyze the data compared to actual closing of the leads. If you have an open field, you may get answers like this:
100 Cds
100-200 DVDs
one hundred
5000
That will require a lot of manual type of analysis.
Depending on how your PPC campaign and analytics are setup, if you did ranges (just using these numbers for illustrative reasons):
0-100
101-500
501-1000
1001-3000
3001-5000
More than 5000
You may be able to track keywords back to potential size of lead and eventually to conversion. This would take some work to do, but if the PPC campaign is large, it might be worth doing.

Yes, this landing page is much better than before, now there is high chance to get more conversion. You should maintain your CTR for high quality score and to get low CPL.
