cookie popup troubleshooting

Hi, I installed a cookie popup for the first visit in my site, i use golive6....It works on my computer but once uploaded, it does not appear, eve worth, i cannot even open the source page in my browser.

Could someone have a look at my coding as i cannot find what is wrong alone.
