I am using Adwords for the first time. How do you determine how much you will pay for a click? It appears that some people are paying $45 per click, and I must be missing something.. $45 per click is crazy. Even $1 seems like a lot. Can someone give me some advice on this?

I guess you already visited http://adwords.google.com
From there you can start sign-in process and check bid prices. You don't have to actually finish the sign-in process.

$45 sounds crazy to me, and I spent thousands on Google in the last 4 years.
I believe that "mesothelioma" could be $100. Look who's bidding on it... Organizations that hope to make thousands on each patient, and lawers that hope to make millions.
Pay as much as you feel comfortable. If you spend $1 per click, which brings on avarage $50 for every 50 clicks -- you only waste your time. If your ROI shows that you make more money, you can probably afford to spend more as well.

I hope I don't violate your rules. I am new. I just start at a very low rate and work my way up. This way I see where I stand on AW under a certain term and if I need to increase the dollar amount I do. Start low go high. Unfortunately too many people do the opposite. No dollar is too much if you are making money off of the bid. Find a lower searched for term and be #1 on AW for it. This works better.
