I've read a number of articles and posts about keyword density. I've seen recommended ranges of anywhere between 1% and 15% for your page.

Generally speaking, when an article quotes a percentage, say 3% density for example, does this usually include all occurences of the keyword i.e. title tag, description, body, url link text etc., or is it just reffering to the visible text?

I don't worry so much about keyword density. I have over 1,000 articles on my websites with keyword densities ranging from around 2% to 40% or more. All seem to do equally well with the search engines, Google included.

IMO the best policy is simply to write copy that is useful and easy for the user to read and understand. The keyword density will take care of itself.

I have discovered over the years that there is a small handful of SEO techniques that provide the most bang for the buck, and keyword density isn't one of them.

That being said, keyword density includes everything in the code: text, image ALT tags, links, etc. - In other words everything that the SE robots can read and understand.
