I also thought that the spellcheck when I tried to send was annoying (trying to correct every field name). Those would all be obstacles to getting a form submission sent to you right now.
Couple of questions about the form:
1) You ask for birth year AND age group. Wouldn't one or the other do what you need?
2) Since this will be sent by their email client, why do you need to ask for email address?

I saw this form on another site and thought it would be good to have. What to put into this form or take out I don't have a clue. I have been messing around with it for a few months or more.
What I would like to do is as my users send in this info I can make sure that what they ask for is what they get. If all 26 year old want say an itune than I can only send that group what they want and not have to bother the rest.
As for there email. Some users have more than one and they might say send to this one and not the other.
I am sure that there are more simple mailing programs or scripts out there. What to use I don't know. What I do know is I want something that is KISS. Keep It Simple Stupid.
I thought this one was that.
As for the spellcheck. You got me. I was not awear that it had one. What to do?
I will have to put that info on the submit button. Thanks for pointing that out.
