How do you make forms?

I need a form for my "contact us" Page but I don't know how to make forms. I found a free form making site but after a customer submits a request it gives pop up advertisements for thier site. I also want to know how to customize it like radio buttons, check boxes, ect. Please I need any comments or suggestions you may have! Or if you know of a free form making site that does not advertise for thier site on the form please let me know. Thanks!

First of all I would never suggest using a third-party(remotely hosted) script, especially for formmail, guestbooks, etc.
As you've just mentioned, advertising, etc. It's garbage

There's plenty of free scripts readily available.
What programming flavour is available to you?
ie) php, perl, etc.

Check the user tools section at your web host. Most decent web hosts offer ready-made contact forms that you simply cut-and-paste into your page.

Most of the hosting companies that are not free will offer some type of form processing. There are a lot of pretty cheap hosting sites - I always recommend godaddy for domain registration, they might be pretty good at hosting. They seem to be pretty reasonable. And then you can use a form handler there.

Link is to the PHP forms, which seem to work the best for me, plus they are easier to modify and read than the perl ones :)

There are several ways to create a form, but they are all dependant on the hosting solution (ie. asp, php, coldfusion, etc.)

If you are using php, then you already have some direction from previous posts. If not, perhaps you can indicate what langauge you are developing in, so we can direct you to a specific link.
