simple script help

Could anyone please tell me if you know of a script which can do the following:
You have text assigned numbers 1 to 5 on a page
when you click on say number 1, you would be redirected through a script (php or perl for instance) to a url associated to that number
Number 2 would redirect to a different url address associated with the number 2 in the script.
and so on through all the rest of the numbers.
so to sum up.
the first page would have numbers on it.
a script say php or perl would redirect the number when clicked on to a URL address assigned to that number.

Hopefully the script would include admin facilities, to add more URL re-directions to the numbers associated with on the other page from within website.
I hope you can understand what I am looking for from the above.
