Default Form Error - Help Please!!!

Hey Guys/Gals,

I'm simply looking for a little assistance. You guys are always so helpful, so I thought I would give it a shot. I'm trying to set a form up on my website, however it fails while trying to process. I'm not sure where I'm going wrong. I'm getting:

Internal Server Error
The server encountered an internal error or misconfiguration and was unable to complete your request.
Please contact the server administrator, and inform them of the time the error occurred, and anything you might have done that may have caused the error.

More information about this error may be available in the server error log.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

Any ideas what the problem may be? I use CPanel, and I've uploaded the script to my cgi-bin, and set the permissions, however she just doesn't wanna work... Any advice is much appreciated. Thanks in advance.

Want to throw us a bone or two here? Not much to go on with what you listed.

A Url to the form you are using would be nice. Plus what script are you using -- can we find it on the net somewhere so we can look at it in detail?

Sorry about that.. It was a long night last night.

My Server's Perl Version is 5.8.1

I'm using a program called Visual Form Mail to generate my scripts.

The Form can be found at http://www.1stopforgolf.com/hernando_survey.htm

Any help would be great. Thanks for replying!!!

I know nothing about Perl, but the file to process your form at http://www.1stopforgolf.com/cgi-bin/hernando_script.cgi does not exist. You will need to verify that you installed your script correctly to start.
