Nice and Clean.

Since your site is also a link exchange board, I would add a separate “Link to us” page displaying various banners and/or ready to use html code for linking to your site. And placed a link to this page in ".. Us" menu on top-right.

Thanks for your comments.

The way it works at the moment, when somebody actually submits a link exchange request, they get shown a confirmation page.

On that confirmation page, I have included instructions and code for adding a link to our site. I'm not sure if also need what you suggested.
