Please review my website and give your comments.

Bariatric Support Group - Bariatric Surgery, Gastric Bypass Surgery - Cartagena, Colombia.
This is a medical website regarding weight loss surgery or gastric bypass. Please tell what you think about the design, layout and if the website gives the idea I want to express with it.

Also if you can give me suggestions to increase the number of people filling my contact us form, I mean the conversion rate.

You've got a nice clean layout with a lot of great information. With a few changes, I'm sure it'll be even better.

Your content column, particularly the paragraph text, needs some padding on the left side where it tends to touch the left column.

While the meta "author" tag is completely redundant and should be removed, the others are empty. Meta description and keywords tags are still very important, so you'd do well to populate them properly.

Your page title needs revision as well. "Bariatric Support Group" is the least valuable component. Use the page title to focus on the keyphrase and keywords most likely to be searched with. "Gastric Bypass Surgery", for example. You Can put "Bariatric Support Group" in the site description if you must.

I read the very first paragraph and you lost me...

I've bolded a few of the issues that you need to address. The site content makes users think it's a more of a vacation spot, than it is a reputable provider of "world class" medical care. There appears to be a language issue as well, the translation to English does not read well. I think most would agree that bad grammar is simply not indicative of a professional medical institution.

"Bariatric Surgery", the site seems to focus on that but I wonder if it's the term most prospective patients would use to search with.
"Cultural patrimony", you're talking over the heads of your users.
"warm, clear Caribbean waters", is this a vacation destination?
"Colonial Architecture", what does that have to do with medical care?
"patients operated around the world", this sentence utilizes poor grammar and needs revision.

Your opening paragraph could be much more effective were it to focus more on the site purpose than on outlining a wonderful choice for a holiday. Use it to define a couple of medical terms, integrate some keywords and keyphrases that people are most likely to search with.

On your home page, I think you might want to replace the image of the obese women sitting on the bench. I can't see that anyone would find that appealing, especially as the first image in your home page content. Same goes for the Flash animation with the obese man walking by the pool with the girl, simply not the best choice. You might consider using those photos on another page, for example, the page which defines "morbid obesity".

I'd recommend that the home page focus solely on building the the BSG's image as medical professionals, what they do and why they're the best choice for anyone considering surgery of this kind. You might also want to include some actual patient testimonials and their success stories.

Being that there are so many "fly by night" outfits around, you'll need to work a little harder to establish the BSG's credibility. Your "Our Doctors" page provides credentials and that's great, but you'd do well to build personal relationships with your prospective clients.

To do this, describe how each member of the team uses their skills and experience to solve particular issues and problems that their patients have had. This single page is what's most likely to entice patients to contact the BSG. Try to make the team members seem more approachable, personal, caring, understanding and knowledgeable.

For example:

He specializes in (another kind of) Surgery, and has been a part of this surgical group since (year). He has been performing (special procedure) since (year), and now specializes in (another kind of surgery) since (year). He has a special interest in (another area of expertise).
I'd recommend more photos of your medical facilities and accommodations. Try to get some with the Surgeons, support staff and some happy patients in them. Show users what they can expect to see when they get there. This would go a long way toward establishing more credibility and in developing a caring, professional image.

You're selling your services and solutions to people, so you have to build a relationship with them and give them enough information to make them want to find out more.

At the very least, ALL of your contact information should be CLEARLY VISIBLE on your "Contact Us" page. Big, bold and in your face. Users should be able to print that page and have everything they need to find you via phone, fax, email or snail mail. They should have enough information to be able to drive right to your door.

Whom can they contact? Who will answer their questions when they fill out a web form? Can they telephone? Do you speak English? Will the person that answers the phone be able to help them? Where is your mailing address? Why is your telephone number way down at the bottom of the page where people can't see it? It should be right up at the top of the page.

I like the top banner with the photo of the 3 docs.

Your English-language text needs refining.

You need to spend some time tweaking your styles to get everything aligned correctly in both IE and Firefox.
