I would like to know what you think of my design, (I tried to keep it simple) adsense placement, logo, or anything else that might make my site more accessible.

I would recommend you to remove the Google ads that they are above the Form with the URL because the form is the main purpose of the Site. When I entered the Site I thought that it was broken (!) because of the Ads. You might put the Ads under the form because else it's seems more with an Ads Site than a Proxy Site as you said.
