simple help needed for website - database fileds

Hi,

I am looking for someone to volunteer to help me connect 2 database fileds for a petition website targeting the 2008 U.S. election. I have created the fields but for some reason the info is not getting recorded in the database.

Could also use some help with my design to spiff it up to attract more people.
