I have written an online form which needs to be submitted to a database. It is connecting to the database, however it is not inserting the data at all.

I was wondering if someone more experienced with ASP.net/MS SQL could have a look at the code and troubleshoot the database insertion. PM me if you believe that you are experienced enough with ASP.net and database injection that you think you could troubleshoot this rather quickly - time is of the essence in this project.
