If a user has javascript turned off then your form is going to go through with incomplete/invalid data.
If you would rather do it with php and can wait until the weekend then I can write up your form and validate the fields to the necessary format using regular expressions.
Take a look at my site and play with the demos and try inputting invalid/incomplete data and see if thats what you are after.
If you can take care of the basic programming yourself and are just falling down on the validation then I can write up some functions to validate to whatever specifications you have.
