I need a fairly simple enquiry form integrated onto my homepage.

It will have two text fields and a couple of tick boxes.

Small amount of validation required (simply to validate that the right amount telephone number digits are given, and that at least one of two tick boxes are used).

Once submitted the form will need to move onto a 'Thank you for your enquiry page' - which will be provided.

Will need to form to look nice and neat, so a bit of customisation might be needed (nothing too much though).

Form will be delivered to an email address in easy to read format - with time & date if possible.

Can use ASP, PHP or Perl with server.

Looking for a fairly quick job (1-2 days), once I get the go ahead.
