Is there a hard and fast rule in regards to internal linking? Can someone provide an image of a flow chart of how a sites internal links should look, or does it differ between sites.

Internal linking is just a strategy only and it differers from how you design your site whether you design it dynamically or statically and you can place links that are relevant to pages within a certain page with the intention to bring a good link internal structure for your site will help all pages rank and be indexed well.

Mostly, if you do have contents on your index page, you can link your keywords to each related pages on your site. It's just simple as it is.
