Form and Forum SPAM and how to curb it

I'm now constantly getting form spam from allot of our websites as well as forum spam where bots log into even the latest version of PHPBB with image verification on registration, I don't know how they're getting into it, but they are. It's driving me nuts!

I know captcha does a pretty good job of curbing spam form spam. What other means are there to curb form spam?
