You need to have an Adwords account and you can find out details from the help pages over at Google Adwords. The test is multiple choice.

you need to pass some hurdles (e.g., have a client center set up, and manage a certain volume of adwords for a set period of time) before you can take the test. google has the program well defined on their site.
