Are you wanting something like a poll? If you want ASP let me know & I can send you one that we give to our hosting customers for free.

If it is not really a poll, can you maybe explain a little about what you are needing? And ASP or PHP or even .NET?

I forgot to tell you that i need a simple poll,for example,a poll about "what's the movie you prefer" etc.
