mail forward only partially works (case sensitive?)

I have an apache server, and am using a webforward to redirect domain A to domain B and it works fine. I also have a mailforward setup for domain A that I would like to go to a certain hotmail address. I have it setup to forward ALL mail yet some emails work perfectly and some don't. Ie Steve@abc.com works but steve@abc.com doesn't...

Any ideas?
