Without knowing how your "conversions" are defined, or exactly how & where they're being measured. it's difficult to point to any particulare cause.
The most common cause is owing to the relevant script being placed in the dynamically generated portion of a page, rather than in the static area.
Even when in the static area, a user refresh can cause multiple counts.
Also, it is possible on some sites for a user to make, for example, multiple separate purchases, thus leading to legitimate multiple conversion counts per visit.
