All you need to do - presuming you are using IE - is choose 'Page Setup...' from the 'File' menu and delete the contents of the Header and Footer fields.
After you print with these settings, they become default. If you want to reset them, enter

&w&bPage &p of &P

into the Header field
and

&u&b&d

into the Footer field.
