I use to use Duncan Carver's LMA but doesn't look like he's supporting it anymore and the latest version has some bugs (it's encoded so not much can be done). The newest version offers paid links through PP so I wanted to use that feature. PHPLD has it built in and looks like a pretty good community for support. Mod_rewrite built in as well.
