I hate to be the disenter but most of what is posted is misinformation.
Your primary keyword term is the one used first in your page title tag and which reflects what the page content is themed for.
For example if you wrote a page of content on 'search engine marketing' then your primary keyword term would be 'search engine marketing.
This then would be the first word in your page title tag
<title>Search Engine Marketing</title>
Now if your title was
<title>Search Engine Marketing Internet Advertising</title>
The secondary keyword would be 'Internet Marketing'
Going back to the primary term this should also be one of the first set of words used in your meta description.
From there you could / should use it as in h1 tag at the top of your page.
Used once or twice in alt image tag.
Used again in the content with 5% density level.
Use in an h3 tag mid page if feasible.
and then used in the last 25 words or so of your page content.
It should also be a link on your page.
This then would also be the keyword phrase to use in backlinks to the page that you place on other sites or that others use to link to your page.
In closing the primary keyword term should be the one that will drive targeted traffic to the page that focuses on the keyword term.
