Couldn't this hurt accessibility, because the text to speech software has to read alt tags that are not related to the page content?

What type of link are you trying to get? The only incoming link I can think that this would gain is a link from a Search engine image search function, which would be unlikely to generate much traffic, since users that see a design element in the results will likely ignore it, and which would not gain any
SEO benefit since these links are invisible to other search engines.

Could this not trip a spam filter? Especially if you are repeating keywords that are not present in the rest of the document or using the keyword more in the alt text than in the page text surrounding the image?
