Is this the only site you have on this AdSense account? If not, there may be another offending site that caused this.
Also, and I'm trying not to be judgmental, they *may* see that page as a doorway (since IMHO it's just a GTR (Get The Referral) ad page).
If all other factors are cut away (as in this is the only site on the account), you may have been banned because of this bullet from their
Program Policies

Even that isn't much help since there are a large number of sites that in violation of that statement yet still are collecting AdSense rev's.
