I can understand why the designer would need to charge for this additional work. Scaling a website's structure is very close to starting all over. Who is your target audience and what screen size would they most likely have? 1024X768 is quickly becoming the new standard for most of our sites - but as a compromise we generally try to have a little cushion for an 800X600. You will be hard pressed to find a design that looks perfect with any resolution. Fixing a websites central structure is a great way to maintain control and display the site as you intended. There are many scalable website advocates out there who will argue otherwise. Visit their own sites and you'll get your answer ;)
