Re: adwords system meltdown
As someone who spends 4 figures daily on Adwords for our own sites and client sites, I have noticed a change over the past months. It's getting tougher. I sometimes feel the Adwords algorithm has become more complex than the Google directory!
If your keywords are too generic and/or popular sometimes this will trigger them to be put aside for manual checking. Now I have noticed that this does not always get done terribly quickly and so if after a couple of days nothing happens I will put in a call to my Adwords rep.

DoctorSEOSoCal has a point too. Check your campaign spending cap. It may be OK for your existing adgroups, but when adding a new group be sure there is enough slack.
mlevenhagen suggests there may be a cap for popular words. I think he may be right. But I doubt whether you will be OK if you just 'wait your turn'. What is more likely is that the adwords 'Hal Superbrain' is looking at the effectiveness of the rest of your campaign and account, in terms of clickthrough rate and grading you.
Lastly, reduce your in-trial terms. There definitely is a per account cap on these.
